As part of my responsibilities as the adoptive forever home of _____________________,

I agree to the following:
    
  To treat this pet with love and kindness.
    
  To assure scheduled vaccinations and necessary vet care in case of illness.
    
  To license dog.
    
  To microchip dog for identification purposes if lost.
    
  To maintain ownership and not give pet to shelter or even to relative.
    
  To contact WNY Shiba Rescue for assistance or surrender.
    
  To utilize only positive reinforcement training after corrective signals.
    
  To enroll in obedience classes if necessary.
    
  To feed dog nutritious dog food in appropriate amounts for dog size.
    
  To allow provisions so dog may relieve him/herself every 4-6 hours.
    
  To keep dog on a leash at all times unless in a 4 foot fenced yard.
    
  To forgo possession of small animals such as bunnies, ferrets, hamsters, gerbils, or
       anything that is prey to a Japanese hunting dog.
    
  To assure daily exercise.
    
  To purchase appropriate and safe toys with which this dog can play.
    
  To accept home visits from WNY Shiba Rescue to assure care is adequate.
    
  To accept a dog that has been altered and is up to date on vaccines for preventative
       illness.


                                                           Liability Agreement

I do not hold Western New York Shiba Rescue liable for the action of the adoptee since WNY Shiba Rescue cannot monitor, despite this contract, the appropriate treatment of this animal in its new home.
